Manheim Township Ambulance Association cares about protecting its patients' privacy. In the process of providing the products and
services utilized by our customers, we will collect, use and share certain information you provided. This Privacy Policy explains
what information we collect and how we use that information. The Policy also explains how we protect the security and confidentiality
of your information.
COLLECTION OF INFORMATION
We collect and retain the information necessary for us to provide the services you requested
or utilized when calling 911. in that process we may collect non-public information from you as a result of an illness and/or injury
situation. This is done upon contact with you and/or a family member who has an interest in your care and well being.
SHARING INFORMATION
We
may share information with certain non-affiliated companies or individuals, including other providers inquiring about benefits, family
and/or legal representatives acting on your behalf, and to comply with legal or regulatory requirements.
INTERNAL PROTECTION OF INFORMATION
We
restrict access to non-public personal information about you to those employees whom have a need to know. These are for internal audits,
Quality Assurance and Quality Improvement, and Billing purposes only. We maintain both electronic and physical procedural safeguards
to comply with all Federal and State regulations with regards to guarding this information.
DISCLOSURE OF OUR PRIVACY POLICY

is being sent to you for information purposes and may amend and update this policy as it is required. This privacy policy is listed
at www.manheimtownshipems.org. No action is necessary if you elect to access this information electronically. If you would like to
receive this notice by mail, please provide your name, address, and patient number and submit in writing to Manheim Township Ambulance
Association, attn: Privacy Notice, 1820 Municipal Drive, Lancaster, PA 17601-4105.
ACCESS
All patients have the right to access their
PHI at any time. The request must be in writing and must be granted or denied within a window of thirty (30) days. In event of a denial
it must also be in writing in laypersons terms and explain all aspects of the denial. There will be a small charge for access and
copying of information as permitted by law.
